Horse Skin Conditions: Identification and Treatment Guide

Horses are exposed to an enormous range of environmental challenges — wet, mud, insects, sunlight, contact with other horses — and their skin reflects this. Skin conditions are among the most common reasons horse owners call their vet, and many can be managed effectively at home once correctly identified. This guide covers the most frequently encountered conditions with practical guidance on recognition, treatment, and prevention.
Why Horses Are Prone to Skin Conditions
The horse's large surface area, combined with constant exposure to weather, ground conditions, insects, and other horses, creates an environment where skin problems are almost inevitable at some point. Certain anatomical areas are particularly vulnerable: the lower legs, which are frequently wet and muddied; the white-skinned areas that lack the protection of pigmentation; the mane and tail base, which attract insects; and the face, which cannot be rugged or protected easily. Horses kept in poor conditions — wet bedding, muddy paddocks, inadequate insect management — are at significantly higher risk for most of the conditions discussed here.
Rain Rot (Rain Scald)
Rain rot, also called rain scald, is caused by the bacterium Dermatophilus congolensis, which is present in soil and on horses but typically only causes disease when the skin's barrier is compromised by prolonged moisture. It most commonly appears on the back, quarters, and upper neck — the areas where rain sits longest. The characteristic presentation is clumps of matted hair that lift away from the skin like a paintbrush, revealing raised scabs beneath. When the scabs are removed, the skin underneath is moist, pink, and slightly raw-looking. In severe cases the lesions are extensive and the horse may be uncomfortable when the saddle area is affected.
Treatment involves removing the scabs to allow the skin to breathe and dry — softening them first with warm dilute chlorhexidine wash makes this easier and less uncomfortable for the horse. After scab removal, the skin is cleaned and dried, and an antimicrobial topical may be applied. Keeping the horse dry and out of prolonged wet is essential for recovery; light rugging in wet weather protects the back and accelerates healing. Rain rot is self-limiting in most cases once the skin is dry, but severe cases benefit from veterinary assessment.
Mud Fever (Pastern Dermatitis)
Mud fever is caused by the same organism as rain rot — Dermatophilus congolensis — but affects the lower legs, particularly the pastern and fetlock areas. Horses with white legs and pink skin in those areas are significantly more susceptible because the skin is less pigmented and more prone to damage from wet and abrasion. The characteristic signs are matted hair, crusty scabs on the lower limbs, and in more severe cases reddening, swelling, heat, and lameness. The condition can progress rapidly and is notoriously painful for the horse during treatment.
Treatment requires removing the scabs (which can be very painful and may require sedation in severe cases), cleaning the skin, and keeping the legs dry. Prevention is the most effective approach: managing paddocks to reduce deep mud, using track systems or electric fencing to rest wet areas, and considering barrier cream application in very wet seasons before the skin becomes damaged.
Sweet Itch (Insect Bite Hypersensitivity)
Sweet itch is an allergic reaction to the saliva of Culicoides midges — tiny biting insects that are most active at dawn and dusk during the warmer months. Not all horses develop the allergy, but those that do are typically affected for life, with the reaction often worsening with each successive year of exposure. The midge preferentially feeds along the mane, tail base, and belly line, and the resulting intense pruritus (itching) causes the horse to rub these areas compulsively against posts, fences, and any available surface.
The resulting skin damage from rubbing can be severe: broken, thickened, scabbed, and in long-term sufferers permanently changed in texture. The affected areas lose their hair, and the skin may become leathery or corrugated over time. Management focuses on reducing midge contact: purpose-designed sweet itch rugs that cover the entire body including the neck and belly are the cornerstone of management, used from spring through early autumn. Stabling during the highest midge activity periods (dusk and dawn) provides additional protection. Insect repellents designed for horses can help but require frequent reapplication. Veterinary treatment with systemic antihistamines or corticosteroids may be needed during particularly severe episodes.
Ringworm in Horses
Despite its name, ringworm is caused by a fungus rather than a worm — specifically Trichophyton equinum or Microsporum species. It presents as roughly circular patches of hair loss with a grey, scaly, crusty surface that may resemble a ring. The patches most commonly appear on the saddle region, girth area, neck, and face — areas of tack contact and where grooming equipment touches the skin. The infection is spread between horses through shared grooming equipment, rugs, saddle pads, and direct contact.
Ringworm is self-limiting in most horses with good immune function — it will resolve over 6–12 weeks without treatment. However, treatment with topical antifungal washes (enilconazole is widely used) accelerates resolution and reduces spread. All shared equipment must be disinfected or quarantined — fungal spores can survive on surfaces for many months and remain infective. The zoonotic risk to humans is real and should be taken seriously: anyone handling affected horses should wear gloves and wash hands thoroughly. An affected horse should be isolated from others where possible until the lesions are resolved.
Photosensitivity
Photosensitivity reactions occur most commonly in horses with white or pink-skinned areas exposed to sunlight. Primary photosensitivity results from ingestion of photosensitising plants (St John's Wort and buckwheat are common culprits) that make the skin hypersensitive to UV light. Secondary photosensitivity occurs when liver damage impairs the breakdown of a plant pigment called phylloerythrin, which then accumulates in the skin and reacts to sunlight. The affected skin becomes reddened, swollen, blistered, and in severe cases necrotic — and the horse is clearly distressed. Any suspected photosensitivity case warrants urgent veterinary assessment, particularly for the secondary form where liver damage is the underlying cause.
Warts and Sarcoids
Equine warts (viral papillomas) are most common in young horses under two years old and typically affect the muzzle, lips, and occasionally the lower limbs. They are caused by equine papillomavirus and are self-limiting — most resolve spontaneously within a few months. They do not require treatment in most cases.
Sarcoids are a different matter entirely. They are the most common skin tumour in horses and are associated with bovine papillomavirus type 1 or 2, though the exact transmission mechanism between horses is not fully understood. They occur most commonly on the lower limbs, head, groin, and around the eyes and armpits. The significant challenge with sarcoids is their unpredictable behaviour — some remain static for years, while others grow rapidly or transform into more aggressive forms following trauma or incomplete treatment. Any skin lesion that does not resolve spontaneously, is growing, or is in a surgically challenging location should be assessed by a vet. Treatment options include topical chemotherapy (cisplatin or Liverpool cream), ligation with rubber rings, laser surgery, cryotherapy, and immunotherapy — the best option depends on type and location.
When to Call the Vet
Call your vet promptly for any skin condition involving significant swelling, heat, lameness, or rapid deterioration; any lesion near the eyes; suspected photosensitivity; any rapidly growing or bleeding lump; or any condition that is not responding to appropriate home treatment within a week. Uncertain diagnosis is also a valid reason to call — treating the wrong condition can waste time and allow the correct condition to progress.
